Southwest Washington Accountable Community of Health (SWACH) is committed to transforming health outcomes in Clark, Klickitat, and Skamania counties through our robust Policy Advocacy program. We recognize that true health equity can only be achieved by addressing the root causes of health disparities, and we're dedicated to making a lasting impact. Our approach is multifaceted and community-centered:

Our annual Community Needs Survey, open to residents 18 and older, takes about 8 minutes to complete and is anonymous unless you provide contact information. This survey helps guide our programs, funding, and policy priorities, with results shared publicly. Your input is crucial in shaping our community health initiatives, which are funded through various sources including Medicaid innovations and state/federal funds.
