What is the Opportunity?
HealthConnect is expanding our care coordination efforts by increasing our network size. As part of this expansion, we are looking to strengthen care coordination for two priority populations:
Youth/Teens
Individuals Re-entering the Community After Incarceration (Re-entry)
We are seeking to partner with agencies that already have strong experience serving these communities.
This funding opportunity prioritizes organizations that:
Operate programs that work directly with youth or teens (not solely serving parents or families), and/or
Operate programs that work directly with individuals in re-entry
In addition, preference will be given to organizations that serve communities experiencing social and economic barriers to health and services
Agencies whose services are designed specifically for these populations will be considered most aligned with this opportunity.
