The HealthConnect Hub, powered by SWACH, connects health, social service, and community-based organizations through coordinated referral and reporting systems.
Our goal is to improve health equity and streamline access to care for every person in our region.
What an Established Hub Looks Like

HealthConnect Hub ensures that community-based organizations and residents are guiding and advising the Community Care Hub (CCH) on what is best for their community.

HealthConnect Hub delivers ongoing financial, administrative, and operational support that reduces the burden on community-based organizations.

HealthConnect Hub provides standards and operations to ensure the social care network effectively delivers and reports on their services, improves health outcomes, and reduces costs.
