Southwest Washington Accountable Community of Health is fortunate to work with great partner organizations like our HealthConnect Integrated Partnerships. If you are a new HIP employee or supervise a new hire, use these resources to help onboard them with the HealthConnect Hub.

Tasks for New HealthConnect Users

• Complete a New Employee Registration Form for new community-based workers and supervisors at our partner agencies.

• Sign the CHR User Agreement before you access the Community Health Record.

• Report upcoming paid time off to ensure referral coverage at your agency.

• Complete an Offboarding Employee Form for staff leaving a HealthConnect-partner program.

• Log into CCS (Community Health Record).
