CareConnect Washington 2.0

During the COVID-19 pandemic, SWACH’s CareConnect Washington (CCWA) program has provided housing, groceries, and utility support to households with at least one positive COVID-19 test (and in some instances exposure). This program removed many of the barriers individuals faced when needing to quarantine and isolate during the pandemic.

As the public health emergency (PHE) that supported this model has ended, we are reimagining the program to better meet current needs. SWACH is transitioning to an updated version of the CareConnect Washington program, funded by the Washington Department of Health. CCWA 2.0 will officially launch on July 1, 2023. Important changes include:

  • The HealthConnect Hub will no longer provide any procurement starting July 1, 2023. The cut-off for referrals into CCWA 1.0 will be June 30, 2023.

  • If, at any time, the Department of Health reinstates a policy including quarantining and isolating, SWACH will adjust the CCWA program to provide appropriate support. We will coordinate with public health in that circumstance to provide surge support.

  • There will be an overlap period of services into July as we complete referrals that came in by June 30th. Referrals received by the end of this month will still be eligible for current services provided.

  • Community Health Workers who were funded by CCWA will transition to a role supporting HealthConnect Pathways. SWACH will continue to provide funding for these positions to participate in the HealthConnect Pathways program.

    • For CHWs transitioning from CCWA to HealthConnect Pathways, SWACH will provide technical support and training.

Highlights of the original program

SWACH and the Department of Health launched CCWA in March 2021. The original program will have lasted for 28 months when we make these changes. Along with our partners, we were able to have a sizeable impact in our community.
