HealthConnect Network Expansion Opens New Funding Opportunity
SWACH is preparing to expand the HealthConnect Network, creating new opportunities to strengthen care coordination across Southwest Washington. This upcoming funding opportunity will support organizations working with two priority populations: youth and teens, and individuals re-entering the community after incarceration.
Through HealthConnect, SWACH connects people to coordinated, community-based care by bringing together trusted local partners. Through this expansion, SWACH aims to grow the network’s reach and deepen collaboration so more people can access timely, whole-person support where they live.
Michelle Ahmed, HealthConnect Director, shared why this focus is intentional:
“HealthConnect is intentionally focusing on youth and teens, as well as individuals re-entering the community after incarceration, because these populations often face significant social and structural barriers that impact long-term stability and well-being. By addressing needs like housing, food security, employment, and healthcare access during critical transition periods, we have an opportunity to create stronger foundations for long-term stability.”
She also emphasized the value of expanding the network through community partnerships:
“What excites me most about expanding our network is partnering with organizations that bring trusted relationships, deep community expertise, and innovative approaches to helping people access the support they need to truly thrive in their communities.”
The Request for Applications (RFA) will open on March 2, 2026, with a virtual Q&A session scheduled for March 6 at 9:00 a.m. Organizations interested in joining the HealthConnect Network are encouraged to review the opportunity, attend the Q&A, and begin preparing materials.
