What is the HealthConnect Hub?

SWACH’s HealthConnect Hub connects individuals and families with the care and support they need—when and how they need it.
Community-Based Workers help people navigate services, remove barriers, and improve overall well-being.

The Hub is part of the statewide Community Care Hubs of Washington, a network uniting health and social service organizations to strengthen community connections.

How It Works

Need help with health or social services?
Our trained Community Health Workers (CHWs) can help you:

  • Find or establish care with a doctor

  • Manage chronic conditions

  • Access food, housing, or transportation

  • Get connected to local programs and community supports

Getting started is simple:

  1. Fill out the short self-referral form.

  2. A CHW will contact you within two business days.

Our Programs

Southwest Washington Accountable Community of Health’s (SWACH) HealthConnect Hub leads efforts to help people achieve their health goals through tailored programs. Collaborative partnerships promote health equity and help sustain the overall goal of whole-person wellness.

  • The Self-Referral is a free program that connects you with a Community Health Worker who can help you access the resources and services you need to improve your health and quality of life. They can help you with things like establishing care with a doctor, finding support to manage medical conditions, work with you to secure housing, and much more.

  • HealthConnect Pathways is a care coordination model we use to support people based on their specific health and life needs. After a short risk assessment, a Community Health Worker will help you choose the “Pathways” that match your goals and challenges—like housing, health care, or food access—and work with you to complete them.

  • We offer training for Community Health Workers (CHWs) across Clark, Klickitat, and Skamania counties. Trainings cover:

    • Community engagement

    • Cultural awareness and cross-cultural skills

    • Trauma-informed care

    • Social determinants of health

    • Ethics, boundaries, and self-care

  • HealthConnect helps build a stronger health care system by supporting the development of the local workforce and promoting value-based care models.
