Substance Use Disorder (SUD) continues to impact individuals across Southwest Washington. The opioid epidemic is a crisis that has only grown during COVID-19. Care coordination plays a vital role, even more so now, in helping address the epidemic and connecting people to the services they need.

The community-based workforce provides important resources and care to those affected by SUD. We are proud of the work that is being done to address such an important topic - which is why we are highlighting it in this month’s HealthConnect bulletin.

Substance Use Disorder (SUD) referrals are completed when the client/community member attends an appointment with a provider following a referral from a CBW. Unsuccessful referrals occur when a client has not attended an appointment with a substance abuse professional and the pathway is closed.

With 21 out of 29 SUD referrals successfully completed in 2022, SWACH is proud of the work being done by Community-Based Workers across our region. We appreciate your dedication to this critical work.
