HealthConnect Integrated Partners: Growing Our Network

At SWACH, we believe collaboration is essential to creating meaningful systems change in Southwest Washington. The HealthConnect Hub is an innovative approach to breaking down siloes and helping community members receive the support they need. This year, we are expanding existing partnerships and adding new agencies to further strengthen our network.

This year, we are expanding our HealthConnect Integrated Partner (HIP) Agencies from 12 to 17. We are also expanding the number of Community Based Workers to over 82 total. This increase in CBWs comes from both the new agencies we are adding to the hub, as well as expansion within existing partnerships.

Existing HIP Agencies included in the expansion are:

  • Lutheran Community Services
  • YWCA
  • Sea Mar Community Health Centers
  • Skamania County Community HEalth
  • Clark County Fire and Rescue
  • Lifeline Connections
  • Vancouver Housing Authority
  • Outsiders Inn
  • Council for the Homeless
  • Area Agency for Aging and Disabilities
  • SWACH HealthConnect

These partnerships represent a wide array of areas of expertise, cultural and community lived experience, and representation. We are grateful to work closely with these amazing partner agencies and look forward to seeing how their work will continue to expand.
