SWACH Awards Over $1.1 Million to Community-Led Health and Equity Projects Across Southwest Washington
VANCOUVER, WA - Southwest Washington Accountable Community of Health (SWACH) is proud to announce the award of over $1.1 million in grant funding to six organizations working to advance community health, equity, and well-being across our region.
These funds are part of SWACH’s Collaborative Funding Model, which aims to support programs led by and for the communities they serve. Grantees are addressing a wide range of local needs—including culturally grounded wellness, food access, maternal mental health, substance use recovery, and reentry support—through programs designed to meet people where they are.
Award recipients include:
Pacific Islander Community Association of Washington (PICA-WA): Supporting the Wellness Navigation program in Clark County, which connects Native Hawaiian and Pacific Islander communities with culturally informed care, essential resources, and access to traditional foods.
Gorge Perinatal Mental Health Initiative: Enhancing peer-led support groups and services that improve mental health outcomes for new parents, with a focus on serving historically underserved communities in the Columbia Gorge.
One Community Health, Oregon: Expanding services at newly acquired clinics to connect patients with housing, food, and transportation resources, addressing the social drivers of health.
The Sober Place dba Couve Collective: Strengthening the capacity of their recovery café to build community, reduce isolation, and support people in recovery through expanded staff and services.
Nch'i Wana Housing: Extending outreach and support to Native communities in Klickitat and Skamania counties through cultural enrichment and essential resource.
The Foundation WA: Supporting The Restored Transitional Complex, a new transitional housing initiative focused on reentry and healing for formerly incarcerated individuals in Southwest Washington.
Each of these organizations brings lived experience, local leadership, and cultural knowledge to their work—qualities essential to improving outcomes and dismantling barriers to care.
