Community-Based Workers (CBWs) are a vital part of thriving and healthy communities. Known by a variety of names - including Community Health Workers, Peers, Navigators, etc. - These individuals play an essential role supporting community members accessing services and resources to meet their needs.

At SWACH, we are dedicated to supporting CBWs through training, shared learning, and connection across Southwest Washington.

HealthConnect Hub

SWACH’s HealthConnect Hub is a network that connects people and families across Southwest Washington to the care and resources they need, when and how they need them. Through a team of Community-Based Workers, we support community members and advocate for social and health equity. The Hub is part of the larger Community Care Hubs of Washington network, which helps coordinate groups of social and health services across the state, making it easier for people to access support. Our goal is to build healthier, more resilient communities throughout the region.

Ways to get involved

Cohorts

Our cohorts are groups that foster shared learning, resource sharing, and connection to other Community-Based Workers, Community Health Workers, and other related positions across the region. We convene three separate cohorts with different audiences and goals.

  • A collaborative space for community-based workers and supervisors. Meetings cover a wide range of topics related to social services and community-based care. They include trainings, agency spotlights, and opportunities to share resources, events, and network with others in the field.

    When: Every fourth (4th) Thursday of the month, from 9 a.m. to noon.

    Where: Virtual

    Who: Community-based care providers and supervisors.

  • A networking and resource-sharing group tailored to the unique needs of providers in Klickitat and Skamania Counties. Meetings include local trainings, workforce development, and opportunities to strengthen collaboration between agencies and professionals in the region.

    When: Every third (3rd) Monday of every other month, from 10 a.m. - 1 p.m.

    Where: In person, Skyline Hospital

    Who: Community-based care providers and supervisors who serve Klickitat and Skamania Counties

  • A collaborative focused on improving maternal health through shared learning and collective problem-solving. This group supports both community workers and healthcare providers by offering training, sharing best practices, and promoting better outcomes for pregnant and birthing individuals.

    When: Every third (3rd) Monday of every other month, at 1 - 2:30 p.m.

    Where: Virtual

    Who: Community-based care providers, supervisors, clinicians, doulas and others who support birth and maternal health work.

Community Resources

Training & Professional Development

Workforce development is an important aspect of our work supporting CHWs and CBWs. Trainings and professional development opportunities are often available to CHWs, supervisors, and other related fields or positions.

The CHW bulletin board is a way to share upcoming events in real time with your peers. Our bulletin board includes things like:

  • Activities for communities

  • Limited time resources
