Get Connected to Care with HealthConnect Hub’s New Self-Referral Option

Navigating health care and social services can feel overwhelming—but now, getting help is easier than ever. The HealthConnect Hub has launched a self-referral option, allowing community members to directly connect with a trained Community Health Worker (CHW) who can guide them to the resources they need to thrive.

HealthConnect’s self-referral is a free program that helps individuals and families in Clark, Klickitat, and Skamania counties access services to improve health and quality of life. CHWs can help you:

  • Establish care with a doctor

  • Find support to manage medical conditions

  • Secure stable housing

  • Access food and transportation resources

  • Navigate other social and health-related needs

What makes this program unique is its community-centered approach—with many CHWs living and working in the same communities they serve. They understand local challenges, speak your language, and can provide culturally relevant guidance every step of the way.

Getting started is simple:

  1. Fill out the online self-referral form.

  2. A CHW will contact you within two business days to begin working with you.
