A Letter From Our Executive Director

At SWACH, we know that real change begins when community members are heard. It’s one of the most powerful tools we use to address the barriers our communities face. 

In 2024, SWACH’s work was grounded in a simple truth: the people closest to the issues are also the closest to the solutions. Across Southwest Washington, we saw the power that comes from community-led decisions, priority setting, and action. We found this in every aspect of our work – from the new programs we designed, and the advocacy work we supported, to the funding priorities and strategic initiatives we built. A Message From Our Board of Trustees

In times of uncertainty, the strength of our communities in Southwest Washington shines through.

Using Partner Feedback to Shape Our Work in 2025

Our annual Community Partner Survey helps us better understand community need through the lens of organizations and professionals working closely with folks across our region. Through this feedback, we can better understand community priorities and concerns, barriers our partners face, and so much more.

The feedback we gathered in 2024 is helping us shape our work in 2025. Examples of turning this feedback into action include:

  • Providing targeted, strategic workforce development to partner organizations and Community Health Workers (CHWs) in our region.

  • Strategizing funding priorities to help address transportation barriers and the impacts they have, especially in our rural counties (Klickitat and Skamania).

  • Participating in advocacy efforts and initiatives that address housing, recovery, and other policy topics at the local, state, and federal levels.

  • Launching new initiatives that encourage partner organizations to apply for funding to address regional health needs and support capacity challenges.

In 2025, we are also debuting a Landscape Assessment, which pulls data on community strengths and needs. This assessment, which also includes information from our partner survey, will further our understanding of the resources in Southwest Washington, and support our strategies to address health disparities.

  • As an organization, SWACH is split between programmatic and administrative departments.

    Programmatic Departments
    HealthConnect: Part of the Community Care Hubs of Washington, HealthConnect provides a bridge for community members to resources and care across our region.

    Equity & Collaborative Impact (ECI): ECI leads work like regional community engagement, coalition building, strategic technical assistance, policy and advocacy work, and Tribal and Native work.

    Community Health Improvement (CHI): CHI focuses on building up and sustaining rural health networks, and addressing opioid use and substance use disorders (OUD/SUD).

    SWACH’s administrative departments include: Operations, Finance, Human Resources, and Communications.
