The Numbers Behind CCWA 1.0

CareConnect Washington is an innovative program designed to help our community access support and resources when quarantining and/or isolating due to COVID-19. SWACH oversaw this program with support (funding and expertise) from the Department of Health (DOH). The program transitioned to SWACH from DOH in March 2021. Since that time, we’ve been able to support many community members to help slow the spread of the virus.

As part of this month’s HealthConnect bulletin, we are sharing important statistics highlighting the program's success.*

CCWA has supported a diverse workforce of Community Health Workers (CHWs) to support individuals in their quarantine efforts. The program contract funded 12 positions across five agencies. CHWs supported by each agency include:

  • Three from WAGAP

  • Three from SeaMar

  • Two from Share

  • Two from Lutheran Community Services NW

  • Two from YWCA

The pipeline from referrals to enrollment in CCWA was rather successful. Of the 4,663 referrals we’ve received, 3,586 were enrolled and received services.

We were able to support more than 3,300 households with grocery orders through the program, totaling more than $800,000 in support. This led to an average grocery order of $242.42 each.

During CCWA 1.0, we were able to support more than 2,700 requests for housing assistance. This includes mortgage and rental assistance for households who experience positive COVID-19 tests or exposures. Since the program started, SWACH has provided more than $3.5 million in housing support, for an average of $1,296.30 per request.

We are proud of the work that was accomplished with CCWA 1.0. We could not have done this work without our amazing partners - we are grateful for their dedication to our community during uncertain times.

On July 1, 2023, we are transitioning to an updated version of the program - which will include important changes. More information on CCWA 2.0 is available on our website.
